3. Taking blood, doing BPs etc
4. Falls Prevention Programme

Instead of hospital admission, consider:
a) Would Eccleshill hospital admission be a better alternative to hospital admission? Particularly good for patients who need a bit of rehab.
b) Nursing home patients - talk to the doctor who is the 'practice lead' for that home before rushing into admission/treatment.
c) Can your Complex Case Load Manager help co-ordinate care better/keep the patient at home
d) Can CIT (Crisis Intervention Team) help?

Referral Criteria:
  • Has had a fall or is at risk of falling.
  • Is over 65 years of age.
  • Is physically well enough to attend, i.e. blood pressure, angina, respiratory conditions well controlled.
  • Is able to understand and follow instructions. Is fully weight bearing.
  • Is mobile with/without walking aid, independently or with minimal assistance.
  • Understands the implications of group sessions and is able to commit to twice a week for 12 weeks.
  • The referrer feels that their patient will benefit from the group.
  • The patient is able to travel by car.
